Run the PCM self-test
Next, run the PCM self-test with a scan tool and confirm whether P0128 is present. If P0128 is not present, this cooling-system path considers the system operational. Check coolant level safely
If P0128 is present, let the engine cool before opening the cooling system. Do not loosen the pressure relief cap with the engine running or with the system hot, because hot coolant and steam can come out under pressure. Once it is safe, visually check the coolant level in the coolant expansion tank.
Make the P0128 coolant-level decision
At this point the coolant level decides the branch. If the engine coolant level is within specification, this P0128 path calls for installing a new thermostat, clearing the DTCs, and then watching to see if the code returns. If coolant level is wrong, start the coolant-loss checks
On the coolant-loss branch, start with another visual check for obvious damage or failure. Then, with the ignition off and the engine cooled down, check the level at the degas bottle and adjust as necessary before pressure testing. These trucks use a pressure relief cap on the degas bottle, not a radiator cap. Inspect the cap and degas bottle sealing areas for cross-threading, burrs, a damaged O-ring, or anything else that could keep the system from sealing. If the cap or bottle has a sealing concern, or coolant was expelled through the cap and may have contaminated the gasket, install a new cap and/or degas bottle as needed.
Pressure test logic
For the cap test, the degas bottle cap must hold pressure of 145 kPa +/- 21 kPa (21 PSI +/- 3 PSI). For the cooling-system pressure test, slowly increase the pressure to between 124 - 138 kPa (18 - 20 PSI), then observe the gauge reading for approximately 2 minutes. Do not over-pressurize the cooling system, and do not pump the tester too quickly because that can give a false pressure reading. If pressure drops, inspect for leaks and repair as necessary. If no external leak is found and pressure still drops, continue checking for an internal leak path.

If the system does not show an external leak, inspect the coolant in the degas bottle for transmission fluid. Then remove the transmission fluid level indicator and check the transmission fluid for coolant. If that cross-contamination is present, install a new transmission fluid cooler and repair the transmission as necessary. If those checks are clean, inspect the coolant for engine oil, then remove the engine oil level indicator and check the engine oil for coolant. If engine oil and coolant are mixing, move into engine-side diagnosis instead of treating this as a simple thermostat concern.
Check for combustion gases
If the fluid checks are clean, use a cooling-system combustion gas leak tester, such as UView® Combustion Leak Tester part number UVU560000-R or an equivalent tester, and use it the way the tester maker specifies to check the coolant for combustion gases. If combustion gases are present, move into engine-side diagnosis. If combustion gases are not present, the coolant-loss branch considers the cooling system operational.
Keep the overheating branch separate
If the vehicle is actually overheating, that is a different cooling-system branch, so do not mix it into a straight P0128 warm-up diagnosis unless the complaint or code set points you there. That branch starts with a visual inspection for obvious damage or failure, then checks for non-OEM engine components that may be contributing to overheating. After that, verify there is no front-end damage, no radiator or A/C condenser obstruction like leaves or cardboard, and no missing air deflectors. Then, with the ignition off, check and adjust coolant level in the expansion tank and pressure test the cooling system. If it leaks externally, repair it or install new components as needed. If it does not leak externally, keep going.
Continue the overheating branch fluid checks
Still on the overheating branch, inspect the coolant in the degas bottle for engine oil or transmission fluid. Then drain fuel from the fuel conditioning module and remove the transmission oil level indicator to check for coolant in the fuel or transmission fluid. If coolant and transmission fluid are crossing over, install a new transmission fluid cooler and repair the transmission as necessary. If coolant is in the engine oil, move into engine-side diagnosis. If those checks are clean, test the coolant for combustion gases. If combustion gases are present, move into engine-side diagnosis. If they are not present, check cooling fan clutch operation with the ignition off. If the fan clutch is not operating correctly, install a new cooling fan clutch.
Overheating branch flow and thermostat checks
For the coolant pump check on the overheating branch, start the engine, allow the engine to run for 10 minutes, place the climate control function selector in MAX HEAT, and feel the heater outlet hose. If that hose is not hot, install a new coolant pump. If it is hot, continue to the thermostat operation check. If the upper radiator hose is hot, check temperature gauge operation. If it is not hot, visually inspect the thermostat. If damage is found, remove any foreign material or broken pieces and install a new thermostat. If no damage is found during inspection, do not try to open the thermostat with hot water or another heat source, because that is not an accurate thermostat test and it may damage the part.
